It is helpful to have your forms filled out before you arrive for your appointment. Appointments begin on the scheduled time. If you need 10-15 minutes to fill out forms, your session may be shorter than planned. Please help us stay on schedule by filling these out early. Follow the steps below to get started.

Step 2: Print the 3-page document.
Step 3: Fill out the Health Intake form and sign the privacy policy.
Step 4: IF you are an insurance client, please fill out the Medical Intake as well.
