READ BEFORE BOOKING:
- A valid credit card will be required to prepay for your first two sessions while insurance payout, deductibles and limits are being verified.
- Clinical Massage Treatment only: Neck, Arm, Migraines, Whiplash, Frozen Shoulder, Thoracic Back Pain, Wrist Pain, Elbow Pain.
- I see limited patients on a limited schedule. I hope you understand.
- All sessions are 30/45-minute focused treatment with hot/cold therapy and assessment included, and delivered in 4, 6, 8 week intervals.
- Ongoing treatment is not available. If you are not getting better then massage is not the solution.
- Please have a doctor’s referral in hand and please contact your insurance company for PREauthorization before scheduling.
- I happily accept the following as an In-Network Preferred Provider: Aetna, Premera, Kaiser, First Choice, Great West. Full co-pays are due at the time of appointment.
- I am registered and accept L&I/Work-related injury claims. You pay nothing out of pocket. I will bill for services fees. I have never seen any issue with these payouts.
- Price Norman accepts Regence Blueshield. Please contact him.
- I accept Auto Injury Personal Injury Protection 1st Party Claims on a reimbursable basis.
- You pay session to session at full medical billing rate, and then I bill for your reimbursement.
- You receive a check for what was paid by insurance.
- NOTE: I will not see insurance patients from private coverage (i.e. Premera, Kaiser) that are coming in due to Auto Accident. If your Auto PIP is exhausted, I won’t be able to help you. My experience is that the private insurance won’t pay due to Auto or Work related claims and it takes years for the Auto Insurance Claims to get settled when there are disputes. If I accept your copay I am accepting the risk that I won’t get paid.

YOUR RESPONSIBILITY – By scheduling an appointment, you agree to these terms:
By accepting insurance, we make it easier for patients to receive massage treatment. We rely on our patients to be on time to appointments and responsible for service fees incurred. All copays are due at the time of service. All deductibles and service fee balances due as stated by your insurance company are your responsibility and must be paid within 10 days of you receiving your insurance statement or an invoice from Tammy Foss LMT.
Should your active insurance not pay for your session, you are responsible for the contracted service fees that have been agreed upon between us and your insurance company. Those fees may be less than the amount billed. Following any medical session, always refer to your Eligibility of Benefits statement you receive from your insurance company for your fees owed.
All treatment is provided on a medically necessary basis and only if the referral shows significant need for such service. All medical treatment sessions are billed at $120/hour.
In the event your insurance doesn’t pay, you are responsible for the provider contracted amount which is contract-discounted from the standard rate of $120/hour. Your Eligibility of Benefits document from your insurance company will indicate how much you owe. Any refusal to pay will be reported to your insurance company.
Any bill you receive from Tammy Foss must be paid within 10 days of receipt. Further collection efforts will result in additional fees including but not limited to: certified mail fees, attorney fees, collection agency fees, and $60 for every 30 days the balance is late. All returned checks are charged back to you at $30/each. You are responsible for these fees regardless of your legal actions against others or represented by others.

- Low back and hips – recommend personal training and/or physical therapy
- Plantar Fasciitis – recommend physical therapy, orthotics, good shoes, and stretching
- Knees injuries or ankle injuries – recommend physical therapy and ice
- Ongoing maintenance to reduce stress, tension or referred pain – recommend 60m and 90m services with one of our other wonderful practitioners
- Chronic patterns – recommend personal training or physical therapy, assess your pillow and mattress, assess your desk position, nutrition counseling, and acupuncture
- Fibromyalgia or other fasciitis – recommend 60m and 90m services with one of our other wonderful practitioners and acupuncture
